Tout comme il ne viendrait \u00e0 l\u2019id\u00e9e de personne de raisonnable de courir un marathon sans entra\u00eenement, chaque reprise de la plong\u00e9e en milieu naturel doit se faire de mani\u00e8re progressive, par faible profondeur (zone des 12\u201120 m).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ainsi, en France m\u00e9tropolitaine, les plong\u00e9es du mois de mai sont particuli\u00e8rement \u00e0 risque de d\u00e9c\u00e8s et d\u2019ADD (voir ci-dessous la vid\u00e9o faites avec le CROSSMED). De nombreux plongeurs, s\u00e9dentaires le reste de l\u2019ann\u00e9e, pr\u00e9sument de leur condition physique et descendent trop profond (ex. 40 ou 50 m) sans entra\u00eenement sous pr\u00e9texte que leur brevet le permet. C\u2019est une pratique dangereuse.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Statistiquement, le CROSSMED a constat\u00e9 que la population la plus expos\u00e9e \u00e9tait celle des plongeurs confirm\u00e9s de plus de 50 ans (50&nbsp;m, 50&nbsp;ans), par exemple lors de plong\u00e9es sur \u00e9paves profondes en d\u00e9but de saison.<\/em><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Longs voyages et effet de groupe<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Les voyages de groupe, en train, bus ou autre, favorisent potentiellement la convivialit\u00e9, ce qui est positif, mais \u00e9galement les exc\u00e8s\u00a0: nourriture, boissons, manque de sommei, ce qui cr\u00e9e une <a href=\"https:\/\/www.plongee-plaisir.com\/fr\/repos-physiologique\/\">fatigue physiologique<\/a> qui favorise les ADD.<br>Dans ce cas, Il est d\u00e9conseill\u00e9 de plonger avant d\u2019avoir respect\u00e9 au moins 24 h de r\u00e9el repos, une fois arriv\u00e9 \u00e0 destination.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>\u00c2ge et micro-noyaux gazeux<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Des \u00e9tudes r\u00e9centes<sup>1<\/sup> ont montr\u00e9 que l\u2019\u00e2ge avanc\u00e9 est un facteur de risque d\u2019ADD<sup>2<\/sup>. Arieli sugg\u00e8re que le nombre et la surface des phospholipides augmentent avec l\u2019\u00e2ge en raison des d\u00e9p\u00f4ts suppl\u00e9mentaires de surfactants tout au long de la vie<sup>3<\/sup>, favorisant ainsi la cr\u00e9ation de micro-noyaux gazeux pouvant potentialiser la cr\u00e9ation de bulles lors de la d\u00e9saturation. Cela pourrait expliquer le risque plus \u00e9lev\u00e9 d\u2019accidents de d\u00e9saturation li\u00e9 \u00e0 l\u2019\u00e2ge.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Les conseils concernant la r\u00e9alisation de paliers \u00e0 l\u2019O<sub>2<\/sub> (qui \u00e9liminent bon nombre de bulles et de micro-noyaux gazeux) sont d\u2019autant plus d\u2019actualit\u00e9 afin de r\u00e9duire les risques d\u2019ADD pour les personnes concern\u00e9es.<\/strong><\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\"><sup>1<\/sup> Blatteau, J. Il peut en r\u00e9sulter un haut niveau de bulles (grade 3 ou 4 sur l\u2019\u00e9chelle de Spencer) en fin de plong\u00e9e, avec un risque accru d\u2019ADD.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sur ce sujet, Arieli parle \u00ab d\u2019acclimatation \u00bb : \u00ab une bulle d\u00e9tach\u00e9e emporte avec elle une partie des phospholipides (\u2026). L\u2019acclimatation \u00e0 la plong\u00e9e, que l\u2019on peut observer chez les plongeurs exp\u00e9riment\u00e9s qui courent moins de risques d\u2019accident de d\u00e9saturation et produisent de plus faibles grades de bulles (\u2026), peut \u00eatre li\u00e9e \u00e0 l\u2019\u00e9puisement des phospholipides (\u2026). \u00bb<sup>1<\/sup><\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\"><sup>1<\/sup> Arieli R, Marmur A. PMID: 28320890; PMCID: PMC5371562.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Pr\u00e9conisations<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Que ce soit pour des raisons physiologiques (\u00ab acclimatation \u00bb) ou techniques (reprise de rep\u00e8re, flottabilit\u00e9 et \u00e9quilibre, utilisation du mat\u00e9riel), il est fortement conseill\u00e9 d\u2019effectuer un retour progressif \u00e0 la plong\u00e9e.<br>Par exemple, la Marine nationale (<em>Instruction pour la Plong\u00e9e Humaine 1, 2023<\/em>) pr\u00e9conise :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>deux plong\u00e9es de reprise lorsque la personne n\u2019a pas plong\u00e9 depuis un mois ;<\/li>\n\n\n\n<li>une plong\u00e9e de reprise lorsque la personne n\u2019a pas plong\u00e9 depuis 15 jours ;<\/li>\n\n\n\n<li>dans tous les cas, une augmentation progressive de la profondeur, de l\u2019ordre de +15 m max. entre deux plong\u00e9es (ex. apr\u00e8s une plong\u00e9e de reprise \u00e0 20 m, ne pas d\u00e9passer 35 m \u00e0 la suivante).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Dans le domaine du loisir, cela doit \u00eatre vu comme une indication, sous la responsabilit\u00e9 individuelle de chaque plongeur.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Qu&rsquo;est-ce qu&rsquo;une plong\u00e9e de reprise ?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Il n\u2019y a pas une d\u00e9finition pr\u00e9cise en termes de dur\u00e9e, profondeur et profil.
